We understand our SmartGuardTM auto mode technology can help you with your diabetes management.

If you have limited insurance coverage, or do not have any other coverage for (continuous glucose monitor), we may be able to help.

You may be eligible to access CGM as low as $99 per month.

Steps to access this program

Step 1

Currently using a MiniMedTM 670G or MiniMedTM 770G insulin pump or purchasing a new MiniMedTM 770G insulin pump.

Step 2

You are in financial need, you have limited insurance coverage, or do not have any other coverage for MiniMed's continuous glucose monitor (CGM).

Step 3

Call MiniMed or fill out the form below  to verify your eligibility before placing an order.

Step 4

Once your eligibility is verified, we will help place your 12-month CGM subscription order.

What does it include and how much does it cost?

This is a 12-month CGM subscription access program.

This access program applies to the following products: GuardianTM Link (3) transmitter and GuardianTM Sensor (3) when ordered directly through MiniMed.

Transmitter

  • 12-month warranty
  • $0 transmitter, limit of 1 per customer per year

Sensors

  • 1 box per month for 12 months, limit of 12 boxes
  • As low as $99 per 5-pack box of sensors

Who is eligible for the CGM Access Program?

You are eligible for this program if you meet the following requirements:

  1. You are in financial need, you have limited insurance coverage, or do not have any other coverage for MiniMed’s continuous glucose monitor (CGM).
  2. You are currently using a MiniMed™ 670G or MiniMed™ 770G insulin pump or purchasing a new MiniMedTM 770G insulin pump.

The following devices are eligible for the program:

  1. MiniMed™ 670G insulin pump system
  2. MiniMed™ 770G insulin pump system

The MiniMed™ 770G is indicated for people living with Type 1 diabetes, ages 2 and older. 
The MiniMed™ 670G is indicated for people living with Type 1 diabetes, ages 7 and older. 

What do I do if I am not eligible for the MiniMed CGM Access Program?

I have insurance coverage for MiniMed CGM, but my insurance denied my coverage. Will I be able to use the MiniMed CGM Access Program?

If you are fully covered for MiniMed CGM under your insurance policy but your claim was denied for any reason, then you will not be eligible for the CGM Access Program. If you believe that your coverage was denied by mistake, we recommend checking with your insurance provider to understand your benefits. If you are unsure if MiniMed CGM is covered under your insurance policy, call us to verify your eligibility. 1-800-284-4416 (Monday - Friday, 8 AM - 8 PM ET).

What is the process to receive the 12-month CGM Access Program discount?

Please call us before you place an order at 1-800-284-4416 (Monday - Friday, 8 AM - 8 PM ET). When you call, we will verify your eligibility. Once your eligibility is verified and you have signed the CGM Access Program acknowledgment form, we will help place your 12-month CGM subscription order.

You must talk to us about this program before purchasing a transmitter or sensors. This discount cannot be applied to past orders.

What is the cost of CGM with the MiniMed CGM Access Program?

With the CGM Access Program, when you sign up for a 12-month CGM subscription, you can receive a free ($0) Guardian Link (3) Transmitter (if applicable), plus monthly sensor purchases at a price as low as $99 CAD per box for 12 months.

$0 transmitter (12-month warranty), limit of 1 per customer per year.  Retail value $800.

As low as $99 per 5-pack box of sensors (1-month supply), Limit of 12 boxes per customer per year.  Retail value $399 per box.

Eligibility for program and the level of the discount will be based on: (1) household income, (2) household size, (3) and amount of coverage/level of insurance funding.

How will I be billed after I receive the MiniMed CGM Access Program discount?

Once we confirm your eligibility and you have signed the CGM Access Program acknowledgment form, we will help you place a 12-month CGM subscription order.

Your payment card will be charged in monthly installments as low as CAD $99 for 12 months ($300 discount on Retail Price).

Which products are eligible for a discount?

Products that are eligible for this discount are GuardianTM Link (3) transmitter, and GuardianTM Sensor (3).
The following pumps are compatible with the above products: MiniMedTM 770G system and MiniMedTM 670G system.

Program Terms and Conditions Apply:

  • This program has no cash value.
  • This program is for customers in financial need with limited insurance coverage for MiniMed's continuous glucose monitor (CGM) and GuardianTM Sensor (3) to access SmartGuardTM technology with the MiniMedTM 670G or MiniMedTM 770G insulin pump systems.
  • This offer includes 1 free transmitter per year (only if applicable) and 1 box of sensors per month for as low as $99/box depending on financial need.
  • Limited to a maximum of 12 boxes of sensors per year.
  • To be considered for this program, customers must provide documentation confirming that they are not eligible for funding under any government healthcare program and do not have fully funded insurance coverage for MiniMed glucose sensors.
  • Customer must sign up for 12 months of CGM usage. After the 12 months, customers must contact MiniMed to confirm eligibility for renewal.
  • If a customer cancels their 12-month CGM subscription, they must return the transmitter, or they will be charged for the full retail price of the transmitter ($800 CDN).
  • If a customer’s CGM coverage changes during the 12-month subscription, they must inform MiniMed and may no longer be eligible for this program.
  • Customers who have insurance coverage for MiniMed CGM but who have failed to meet medical criteria are excluded.
  • Additional supplies will not be eligible for a discount.
  • This discount cannot be applied to past orders.
  • Most orders will be delivered within 3-5 business days from the purchase date.
  • Customers must verify their eligibility for the program over the phone and sign the CGM access program customer acknowledgement form prior to placing any discounted CGM supplies order.
  • This program is subject to change at MiniMed's discretion and MiniMed may modify, suspend, or cancel the program at any time.
  • Not eligible for any products already covered through fully funded private insurance plans (primary or secondary) or under a government program (funded or eligible).
  • MiniMed reserves the right to refuse any requests where it reasonably suspects that such requests are prohibited by these terms and the above eligibility criteria, any relevant law, or are otherwise deemed inappropriate.
  • Eligibility for program and the level of the discount will be based on: (1) household income, (2) household size, (3) and amount of coverage/level of insurance funding
  • Customers must provide CRA Notice of assessment documents to validate/verify financial need.
