Did you know that coverage for CGM continues to grow?

You may be eligible for MiniMed continuous glucose monitoring (CGM) under your provincial program or through your insurance plan, giving you access to our integrated insulin pump systems, including the MiniMedTM 780G System§ with GuardianTM 4 sensor or MiniMedTM 770G System with GuardianTM Sensor (3) for those under 7.

Nova Scotia coverage

Who:

People in Nova Scotia of all ages living with diabetes that require intensive insulin therapy through multiple daily injections or insulin pump technology and meet the criteria outlined in the provincial program.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM Sensor 3 with MiniMedTM 770G System for those under 7.

How:

Visit the provincial program website to review the eligibility of the Nova Scotia provincial program. Reach out to your Diabetes clinic to review the program and process.

You can receive your MiniMed CGM at the pharmacy.

New Brunswick coverage

Who:

People in New Brunswick of all ages living with type 1 diabetes or living with type 2 diabetes that require intensive insulin therapy through multiple daily injections or insulin pump technology and meet the criteria outlined in the provincial program.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM Sensor 3 with MiniMedTM 770G System for those under 7.

How:

Visit the provincial program website to review the eligibility of the New Brunswick provincial program. Reach out to your Diabetes clinic to review the program and process.

You can receive your MiniMed CGM shipped directly to your door.

Newfoundland and Labrador coverage

Who:

People in Nova Scotia up to age 24 living with type 1 diabetes who are using an insulin pump with hybrid closed loop technology capability or who are deemed medically necessary by their diabetes team and meet the criteria outlined in the provincial program.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

To participate, email NLCGMP@easternhealth.ca and request an application package. Reach out to your Diabetes clinic to review the program and process.

You can receive your MiniMed CGM shipped directly to your door.

Prince Edwards Island coverage

Who:

People in PEI of all ages living with diabetes that require insulin through injections or through insulin pump technology when prescribed by a physician or nurse practitioner and meet the eligibility requirements.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

Your MiniMed CGM sensors and Transmitters will be available through the community Pharmacy as part of the Diabetes Glucose Sensor Program.

Ontario coverage

Who:

Eligible residents of Ontario living with T1D will no longer have to pay out of pocket for their MiniMed CGM technology through the Assistive Devices Program (ADP). 

Eligible individuals include those living with type 1 diabetes who are at risk of severe hypoglycemia or who are unable to recognize, or communicate about, symptoms of hypoglycemia. For more information, please visit the ADP website.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

You can check your eligibility through the ADP website and reach out to your specialist physician or nurse practitioner*. Once approved, we will ship you your MiniMed CGM directly.

*Registered as a prescriber with the ADP-registered Diabetes Education Program.

Manitoba coverage

Who:

Residents of Manitoba, all ages, living with type 1 or type 2 diabetes currently on both basal and bolus insulin or using an insulin pump, and meet the criteria outlined in the provincial program.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

Visit the provincial program website to review the eligibility of the Manitoba provincial program. Reach out to your Diabetes clinic to review the program and process.

You can receive your MiniMed CGM at the pharmacy or shipped directly to your door.

Saskatchewan coverage

Who:

Insulin requiring residents of Saskatchewan up to 18 years of age who meet the program eligibility criteria.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

You can review the eligibility criteria on the provincial website.

Review with your Diabetes healthcare provider. Once approved, you can receive your MiniMed CGM from your pharmacy or directly from us.

Alberta coverage

Who:

Alberta residents of all ages living with diabetes that require intensive insulin therapy through multiple daily injections or insulin pump technology, who are enrolled in a government-sponsored health benefit plan and meet the eligibility requirements.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

You can review the eligibility criteria on the provincial website. For residents 18 years of age or older, your doctor or pharmacist will need to complete the following Special Authorization form.

Review with your Diabetes healthcare provider. Once approved, you can receive your MiniMed CGM from your pharmacy or directly from us.

Yukon coverage

Who:

Yukon residents of all ages living with T1D no longer have to pay out of pocket for their CGM technology provided they meet the benefit criteria. 

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

Yukon residents living with Type 1 diabetes who meet the benefit criteria must register with the Chronic Disease and Disability Benefit program at (867) 667-5092. People aged 65 or older can contact the PharmaCare Program for more information at (867) 667-5403. You can view the eligibility on the provincial program website.

Once approved, you can receive your MiniMed CGM directly from us. 

NIHB coverage

Who:

Individuals of all ages living with Type 1 diabetes or people aged 19 and under managing their diabetes with intensive insulin who meet the NIHB criteria.

What:

MiniMed CGM integrated with your insulin pump, including the GuardianTM 4 sensor with MiniMedTM 780G System, or the GuardianTM 3 Sensor with MiniMedTM 770G System for those under 7.

How:

CGM sensors can be ordered by a Doctor or a Pharmacist only through your local pharmacy by using the limited use form and calling the Pharmacy benefit line through Drug Exception Centre at 1 (800) 580-0950.

Learn more about the MiniMedTM 780G system

The MiniMed 780G insulin pump is indicated for use by patients age 7-80 years with Type 1 diabetes, whose total daily dose of insulin is 8 units per day or more. 

The MiniMed 770G is indicated for people living with Type 1 diabetes, ages 2 and older.
