HIPAA MARKETING AUTHORIZATION FOR USE OF PROTECTED HEALTH INFORMATION

MiniMed* respects the privacy of all our customers. With your permission, we may use your protected health information, including your contact information and health-related data, for marketing purposes, promotional offers, survey opportunities, and advertising that we think may be of interest to you. We may also share your information with MiniMed representatives and contractors.  

Please note that this Authorization is optional, and you may refuse to opt in.  If you do not want to opt in, it will not affect your treatment, health insurance eligibility, or benefits, and you will not be stopped or prevented from accessing the CareLinkTM Personal system, or other MiniMed products, mobile apps, or services. You will be provided with a copy of this Authorization after it is acknowledged.

This Authorization will expire ten (10) years from the date you submit this authorization online but may expire sooner if required by applicable law, or if you remove it by following one of the methods below:

    1. Email takecontrol@medtronic.com
    2. Sign in to the Profile page and update your communication preferences
        at www.Diabetes.shop/communication
    3. Contact Customer Service Center at 800-646-4633

Opting out will not affect actions MiniMed has already taken based on this Authorization. Information disclosed pursuant to this Authorization may be re-disclosed by the recipient of the information and may no longer be protected by federal privacy standards.
