Tips:

Once you sign in, you’ll be asked a few questions, similar to what you would be asked if you were to give us a call.
Have your sensor serial number or lot number ready before beginning the request.
Once your replacement order has been placed, you will receive follow-up communications with shipment tracking details.
Please take note of estimated shipping timeframes for your replacement sensor:
- For Simplera^TM, Simplera Sync^TM, and Instinct sensor, made by Abbott, these will ship within 5 business days or less.
- For Guardian^TM 4 sensor and Guardian^TM Sensor 3, these generally ship within two weeks.
If you have less than two weeks of sensors and need a replacement sooner, call 1-800-646-4633 and ask to speak with Technical Support.
