PROGRAMS TERMS AND CONDITIONS:

1. General Pump Pathway Program

  • Customers with more than six months remaining on their warranty are eligible for the Pathway Program.
  • The Pump Pathway Program is for hardware pump upgrades only.
  • A $399 payment will be required upfront to utilize the Pump Pathway Program. The program price includes one (1) insulin pump. CGM products and consumables are not included.
  • The Pump Pathway Program does not extend the warranty period of the replaced product. The customer’s current warranty timeline will not change by purchasing a pump through this program.
  • MiniMed™ pumps have certain labeling restrictions that may exclude certain populations, including age and type of diabetes. We recommend you consult your healthcare provider regarding indications for use.
  • Customers should verify that their insurance provider will support transitioning to the new pump, as it could affect ongoing coverage of their future pump supplies.
  • Customers must return their old pump within 30 days. Customers who do not return their pump will be charged $3,200.
  • Medicare customers within the 13-month rental period on a MiniMed™ 630G or MiniMed™ 670G pump can participate in this program.
  • An ABN will be required to utilize the Pathway program if the Medicare customer is within a 13-month rental period and transitioning from one pump to another.
  • Customer agrees that the amount paid to Medtronic is commercially reasonable considering the approximate value of the exchanged pump, the price of the new pump, and the relationship between the parties.
  • Customer agrees to fully and accurately report in submissions to federal healthcare programs all discounts provided by Medtronic and, upon request by the Secretary of the U.S. Department of Health and Human Services or state agency, make available any information that Medtronic may provide concerning the discount.
  • A valid prescription will be required to purchase a new pump. Other restrictions may apply.

2. Switch2System Program

  • Customers with more than six months remaining on their competitive pump warranty (or 5-year Medicare guideline) are eligible for the Switch2System Program.
  • A $499 payment will be required to use this program. This price includes one (1) insulin pump, one (1) compatible transmitter, one (1) month of consumables and one (1) month of sensors.
  • Customers enrolled in federal, state, or government-funded healthcare plans are excluded from receiving a transmitter and sensors from this program and cannot participate in a payment plan.
  • An ABN will be required to utilize this program for all Medicare customers.
  • Customers may return product that has been shipped as part of this program up to 90 days from the date of shipment.
  • Customers must return their competitor pump or receiver within 90 days. Customers who do not return their pump will be charged $4,500.
  • This program does not extend the warranty period of the replaced pump. The customer’s current pump warranty timeline will not change by purchasing a pump through this program.
  • Customers should verify that their insurance provider will support transitioning to the new pump, as it could affect ongoing coverage of their future pump supplies.
  • Customer agrees to fully and accurately report in submissions to federal healthcare programs all discounts provided by Medtronic and, upon request by the Secretary of the U.S. Department of Health and Human Services or state agency, make available information any information that Medtronic may provide concerning the discount.
  • MiniMed™ pumps have certain labeling restrictions that may exclude certain populations, including age and type of diabetes. We recommend you consult your Healthcare Provider regarding indications for use.
  • A valid prescription will be required to purchase a pump, CGM and supplies. Other restrictions may apply.
  • Medicare customers within a 13-month rental period cannot participate in this program.
  • Customers using Kaiser health plans are excluded from this program.

3. MiniMed Pump Trade-in Program

  • This program is available for out-of-warranty MiniMed™ insulin pump customers as well as out-of-warranty competitor pump customers.
  • A sales credit may be applied to orders where a customer is upgrading from an older model pump to a newer model pump through insurance.
  • Customers may receive a $1,000 credit for trading in a MiniMed™, Tandem®, Animas®, or Roche® pump. Customers may receive a $750 credit for trading in a Omnipod® receiver.
  • Customers must return their old pump within 30 days. Failure to return your old pump will disqualify you for the Pump Trade-in Program credit, and you will be responsible for the full estimated cost of the Medtronic pump system being purchased.
  • MiniMed™ pumps have certain labeling restrictions that may exclude certain populations, including age and type of diabetes. We recommend you consult your healthcare provider regarding indications for use.
  • A valid prescription will be required to purchase a new pump. Other restrictions may apply.

4. Zero Pathway Program

  • Customers are eligible to participate in the Zero Pathway Program if they have an in-warranty MiniMed™ 770G, MiniMed™ 670G, or MiniMed™ 630G insulin pump & have more than six months remaining on their pump warranty.
  • The program includes one (1) MiniMed 780G™ insulin pump, two (2) Instinct sensors, made by Abbott. This program is not available for any other CGM options that Medtronic Diabetes may offer. 
  • Customers whose payers utilize a monthly CGM supply allowance are only eligible to receive a MiniMed™ 780G pump and cannot receive Instinct sensors as part of this program.
  • The Zero Pathway Program does not extend the warranty period of the replaced product. 
  • Customers must return their old pump within 90 days. Customers who do not return their pump will be charged $3,200.
  • Customers who do not purchase supplies from Medtronic Diabetes should call Medtronic Diabetes to participate in the program. 
  • Customers with commercial insurance residing in Massachusetts, Michigan, Rhonde Island, & Minnesota, and/or customers enrolled in any federal, state, or government-funded healthcare are excluded from this program.
  • Customers enrolled in Kaiser Permanente are excluded from this program.
  • A valid prescription will be required to purchase a new pump and/or CGM. Other restrictions may apply.
  • Customers should verify that their insurance provider will support transitioning to the new pump and sensor, as it could affect ongoing coverage of their future pump supplies.

5. General Continuous Glucose Monitor (CGM) Pathway Program

  • The Continuous Glucose Monitor (CGM) Pathway Program is for CGM customers who are using a MiniMed™ 630G, MiniMed™ 670G or MiniMed™ 770G pump. CGM sensors are not included and are sold separately.
  • Customers must have a Medtronic in-warranty transmitter to participate in this program.
  • Customers using this program to upgrade to Guardian 4 CGM will receive (1) Guardian 4 transmitter at no cost.
  • The Program does not extend the warranty period of the replaced product. The new transmitter will inherit the warranty of the replaced product.
  • Labeling restrictions for MiniMed™ CGM may exclude certain populations, including age and type of diabetes. We recommend you consult your healthcare professional regarding indications for use.
  • A valid prescription will be required to obtain a new CGM. Other restrictions may apply.
  • Customers should verify that their insurance provider will support transitioning to the new CGM, as it could affect ongoing coverage of their future CGM supplies.
  • Customers with commercial insurance residing in Massachusetts, Michigan, Rhode Island, Minnesota and/or customers enrolled in any federal, state, or government-funded healthcare are excluded from this program.
  • Customers enrolled in Kaiser health plans are excluded from this program.
  • Customers whose payers utilize a monthly CGM supply allowance are excluded from this program but can upgrade to Guardian™ 4 CGM on their next order.
  • Customers are not required to return their old transmitter.

6. Guardian™ Connect System Smart Step Pathway Program

  • The Guardian™ Connect System Smart Step Pathway Program is for Standalone Guardian™ Connect System customers transitioning to a Medtronic sensor-augmented insulin pump system only. CGM supplies and consumables are not included.
  • Customers must have an in-warranty transmitter to participate in the Guardian™ Connect System Smart Step Pathway Program.
  • This program is available to customers who have insurance coverage for the Guardian™ Connect System.
  • Customers transitioning from one Medtronic CGM system will not have any upfront out-of-pocket costs to Pathway to another Medtronic CGM transmitter.
  • Upon completion of the transaction, customers will receive one (1) transmitter that is compatible with their current pump system.
  • The Guardian™ Connect System Smart Step Pathway Program does not extend the warranty period of the replaced product. The customer’s current warranty timeline will not change by purchasing a CGM through this program.
  • MiniMed™ CGM’s have certain labeling restrictions that may exclude certain populations, including age and type of diabetes. We recommend you consult your Heathcare Provider regarding indications for use.
  • Customers should verify that their insurance provider will support transitioning to the new CGM, as it could affect ongoing coverage of their future CGM supplies.
  • Customers must return their old CGM within 30 days. Customers who do not reutn their CGM will be charged $200.
  • Customer agrees that the amount paid to Medtronic is commercially reasonable considering the approximate value of the exchanged CGM, the price of the new CGM, and the relationship between the parties.
  • Customer agrees to fully and accurately report in submissions to the federal healthcare programs all discounts provided by Medtronic and, upon request by the Secretary of the U.S. Department of Health and Human Services or state agency, make available any information that Medtronic pay provide concerning the discount.
  • A valid prescription will be required to obtain a new CGM. Other restrictions may apply.
  • Program terms and conditions are subject to change at any time and without notice.

7. Continuous Glucose Monitor (CGM) Supply Allowance Process

  • The Continuous Glucose Monitor (CGM) Supply Allowance Process is for CGM upgrades only. CGM supplies are included as prescribed.
  • Customers transitioning from one CGM system will not have any upfront out-of-pocket transmitter cost for the opportunity to transition to another CGM transmitter.
  • Applicable out-of-pocket CGM supply costs will apply.
  • Upon completion of the transaction, customers will receive one (1) transmitter that is compatible with their current pump system along with their monthly one (1) 5-pack of compatible sensors and other CGM supplies as prescribed.
  • This process is available to customers whose payers utilize a monthly CGM supply allowance. Customers with insurance providers that bill transmitters and sensors separately are excluded from this process.

Important Safety Information: MiniMedTM 780G system with SmartGuardTM technology with Instinct sensor, Simplera SyncTM sensor, and GuardianTM 4 sensor

The MiniMedTM 780G system is intended for the continuous delivery of basal insulin at selectable rates and the administration of insulin boluses at selectable rates for the management of type 1 diabetes mellitus in persons 7 years of age and older, and of type 2 diabetes mellitus in persons 18 years of age and older requiring insulin. The system is also intended to continuously monitor glucose vales in the fluid under the skin.

The MiniMedTM 780G System includes SmartGuardTM technology, which can be programmed to automatically adjust insulin delivery based on the continuous glucose monitoring (CGM) sensor glucose values and can suspend delivery of insulin when the sensor glucose (SG) value falls below or is predicted to fall below predefined threshold values. The system is intended for use with connected sensors, including the Simplera SyncTM and GuardianTM 4 sensors and integrated continuous glucose monitors, including the Instinct sensor, each of which has different wear-time, form factor, insertion site, and other distinguishing characteristics that relate to sensor performance. Consult the appropriate sensor user guide when using the system. Discuss treatment decisions with your HCP.

WARNING: Do not use the SmartGuardTM feature for people who require less than 8 units or more than 250 units of total daily insulin per day. A total daily dose of at least 8 units, but no more than 250 units, is required to operate in the SmartGuardTM feature.

WARNING: Do not use MiniMedTM 780G system until appropriate training has been received from a healthcare professional. Training is essential to ensure the safe use of MiniMedTM 780G system.

WARNING: Do not use SG values to make treatment decisions, including delivering a bolus, while the pump is in Manual Mode. When the SmartGuardTM feature is active and you are no longer in Manual Mode, the pump uses an SG value, when available, to calculate a bolus amount. However, if your symptoms do not match the SG value, use a blood glucose (BG) meter to confirm the SG value. Failure to confirm glucose levels when your symptoms do not match the SG value can result in the infusion of too much or too little insulin, which may cause hypoglycemia or hyperglycemia.

Pump therapy is not recommended for people whose vision or hearing does not allow for the recognition of pump signals, alerts, or alarms. The safety of the MiniMed™ 780G system has not been studied in pregnant women or in persons using other anti-hyperglycemic therapies that do not include insulin.
